Asian Blepharoplasty
A young male patient presented with monolids, mild brow ptosis, and prominent lower eyelid bags. His primary goal was to achieve a natural-looking upper eyelid crease while maintaining a masculine appearance. He was not interested in undergoing a brow lift and wished to preserve his asian features and avoid an overdone or artificial result. In the same procedure, a lower blepharoplasty was performed using a scarless transconjunctival approach to improve the under-eye fullness.

Presentation & Anatomical Deficit
The patient’s brow ptosis contributed to a heavy upper eyelid appearance, while his monolids obscured the natural contour of the eyes. Creating a defined upper eyelid crease without addressing the brow position required careful planning to avoid an unnatural or excessively high fold. In addition, the presence of lower eyelid bags created a tired appearance despite the patient’s young age. The challenge was to refresh the entire eye area while preserving a natural, masculine look and avoiding visible lower eyelid scars.


The Surgical Plan
A conservative and individualized upper blepharoplasty was performed to create a subtle, natural crease that complemented the patient’s facial features. The surgical plan focused on preserving a masculine eyelid shape while avoiding excessive tissue removal. To address the lower eyelid bags, we performed a transconjunctival lower blepharoplasty, accessing the fat pads through the inside of the eyelid. This approach allowed correction of the under-eye fullness without any external incision or visible scar. Throughout the procedure, meticulous attention was paid to crease design, eyelid height, and bilateral balance to ensure a harmonious result despite the underlying brow ptosis.




Clinical Outcome & Restoration
The photographic timeline demonstrates the patient’s journey from preoperative assessment through surgery, early recovery, suture removal, and long-term follow-up. Mild swelling and bruising were present during the first few days, which gradually resolved. By one year postoperatively, the upper eyelid crease had matured into a soft, natural contour, and the lower eyelid bags were significantly improved. The eyes appeared more refreshed, defined, and youthful, while maintaining the patient’s natural facial identity and masculine features.
